Purposes of processing personal data

When you apply for the maternity package, we use your personal data. We only store and use your personal data for processing your request.

We use the following personal data from you:

• Relationship number
• Date of birth
• First name
• Last name
• Address
• Telephone number
• Email address
• Due date
• Mulptiples if applicable

Purposes of processing and storing your personal data:

• Checking your application
  Your relationship number and date of birth are used to assess your entitlement to a maternity package. If you are entitled to a maternity package, this information and your name and address will be used by us to declare the request to your health insurance company.
• Processing your application
  In order to process your request, we require your name and address. This information is provided to DHL in order to process your request correctly.
• Communication
  To ensure proper communication regarding your request and/or other questions, we use your personal information. Your e-mail address will only be used to keep you informed about the progress of your request.
  
  
• Implementation of statutory duty Natalis
  We comply with our legal obligation to retain your application data within the specified period. Natalis Concepts in Healthcare B.V has chosen to delete your data from our system after 2 years.
  
  

Your information will not be used for:

• Publication
• Advertising
• Newsletters (unless you have given your explicit consent)
• Disclosure to third parties
